SUMMARY Responsible for utilizing advanced clinical knowledge, evidenced-based clinical criteria and extensive background to provide optimal oversight, concurrent review and care coordination functions primarily in the adult setting. Works collaboratively with pulmonologists, primary care providers, facility nursing, HCP’s Hospitalist, Care Management and House Calls teams, health plans and administration throughout the entire trajectory of care to ensure quality outcomes are achieved. KEY FUNCTIONS Consistently exhibits behavior and communication expertise that demonstrate HealthCare Partners’ (HCP) commitment to superior customer service, including quality, care and concern with every internal and external customer. Works with the health care team to prioritize the patients’ care needs and meets with the family and caregivers to discuss care and treatment plan. Identifies and assists with follow-up of high-risk patients when transitioning from acute/post-acute care to the ambulatory setting.
