Peer Recovery Navigator- Early Intervention Services - (Bronx, NY)

Job Description POSITION: Peer Recovery Navigator POSITION REPORTS TO: Manager POSITION SUMMARY: Under the direction and supervision Project Coordinator, The Peer Recovery Navigator (PRN) will assist HIV Testing Counselors with providing pre and post test counseling sessions and community outreach. They will also assist with providing services geared toward assisting men and women who have histories of medical, behavioral and mental health needs. The PRN will provide the peer support to engage the individual on a continuum of care. JOB SUMMARY: Conduct community outreach as part of the mobile testing team to locate clients within the community who are in need of HIV testing, linkage to care or referral services. Conduct Pre and Post Test HIV Counseling Sessions. Responsible for outreach phone calls and/or visits as needed. Accompany clients to appointments as needed. Promote pro-social behaviors.
