Position overview Diversion Navigator assignments include: Works in close collaboration with the Bucks County Department of Mental Health/Developmental Programs to develop, implement, and support the implementation of processes that increase the capacity of system partners to adopt and advance vision of individuals living in community housing supported by natural and community-based mental health services Collects and analyzes data on key transformative indices With County partners, develops data driven directions for change Recommends changes to current mental health housing referral process Supports implementation of team process at provider agencies to divert individuals from residential placement and increase opportunities for community based support Reviews all mental health residential referrals forwarded by the Residential Coordinator Collaborates with other providers and stakeholders on an on-going basis Participates in housing stakeholder groups as active member Promotes development of affordable
