RN Home Health Case Manager - (Philadelphia, PA)

We are currently seeking Registered Nurses (RN) to case manage and provide admission and re-visit care for patients in Philadelphia, Montgomery, and some areas of Delaware County. The RN will administer skilled nursing care to patients requiring intermittent or hourly professional care. All services are performed in accordance with the physician’s orders. Requirements: Current PA license, Baccalaureate or Associate’s degree in nursing or a diploma from an licensed institution or nursing and current CPR certification. Min of one-year home health background is preferred. KeystoneCare is a Medicare Certified, CHAP Accredited, DPW licensed, independent home care and hospice agency.
