Patient Navigator - (Austin, TX)

Job details The Patient Navigator works closely with patients to minimize no-show appointments, maximize patient retention, and support medication adherence. Additionally, s/he is the liaison between the clinic and pharmacies, insurance companies, and patient assistance programs. The Patient Navigator advocates for the patient to ensure that the patient receives all medication(s) prescribed by the patient’s provider. Duties and responsibilities · Coordinates patient appointments and maintains a system for recording patient follow-up appointments, tracking no-show appointments, and monitoring the number of active patients. Keeps up-to-date knowledge on available funding resources for PrEP and other biomedical prevention and treatment interventions. · Coordinates with pharmacies and health insurance providers to ensure that all patients are able to access affordable medication.
