Customer Service Center Team Leader, RN - (Syracuse, NY)

The CSC RN Team Leader is responsible for the oversight and day to day management of the Customer Service Center. Ensures all referrals are appropriate to the requested service line. Communicates with Hospital Home Care Coordinators, Clinical Team Leaders, internal and external referral sources to assure all required details is obtained and confirmed for referral processing. Performance Responsibilities and Standards: 1. Promote continuity of care for referred clients a. Evaluate and process referrals from hospitals, physician offices, skilled nursing facilities, community agencies, patients and their families as needed to assist the CSC RN. b. Answer inquiry calls from clients, families, physician offices, community agencies and insurance companies. c. Supervises Customer Service Center nursing and clerical staff d.
