Infant Service Coordinator - (Petersburg, VA)
Infant Service Coordinator District nineteen Infant Intervention Program is seeking a qualified professional to assume the responsibilities of Infant Service Coordinator. The Infant Service Coordinator is responsible for overseeing all collaborative requirements of service delivery for children birth to age three years who exhibit a developmental delay, atypical development and/or have a diagnosed condition that has a high probability of resulting in a developmental delay. The Service Coordinator is responsible for coordinating and implementing the assessment for service planning, facilitating the development, review and evaluation of the Individualized Family Service Plan (IFSP), coordinating and monitoring the delivery of services, coordinating with medical and other services involved with the child, and facilitating the development of a transition plan for when the child leaves early interventions services.
