Then a Nursing position in Home Health at UPMC Home Nursing Agency is right for you Home Nursing Agency allows you to provide the best care to 1 patient at a time all while they can stay in the comfort of their own home. This is a part time position that will work three 7.5 hour days per week for a total of 45 hours per pay period. There is also an evening and weekend rotation. Responsibilities: Attend compliance training and adhere to the organization standards of conduct, policies and procedures. Coordinates case management activities within a multi disciplinary health care team to insure appropriate delivery of services to patient. Initiates, coordinates and insures ongoing communication with the physician and other members of the health care team. Attends and participates in patient care conferences, team meetings and other related meetings.
