Transition Counselor - (Orlando, FL)

Job Purpose: As the Transition Counselor at Aspire Health Partners, your job purpose is to participate in the overall operation of the Intensive Psychosocial Rehabilitation program and in developing treatment that meet each client's needs. The job functions include - but not limited to - participates in the development of treatment plans, provide direct support individually and in groups to include activities of daily living skills acquisition, support groups and individual counseling when appropriate; maintain the integrity of the clinical record through timely and accurate completion of all required paperwork which includes treatment plans, assessments, progress notes, tracking forms and referrals; develop appropriate rehabilitation curricula for groups and activities scheduled within the program. In addition, this position will assist with Care Coordination for assigned clients, including SOAR applications and other individual client needs.
