Director, Case Management - (Abilene, TX)

Job summary: The Director of Case Management has strategic and operational oversight of the Case Management Department to provide seamless transitions across the continuum of care for Hendrick Medical Center patients. This leadership role requires a highly collaborative, interdisciplinary approach to patient centered care coordination that supports the Hendrick Principles of Performance, Mission Statement, Vision Statement and Core Values. The Director of Case Management is accountable for the Case Management Program, including Case Management, Utilization Management and Social Work. Position qualifications: Min Education: Healthcare related Master's degree. Graduate of an licensed institution of Nursing or Master's Degree in Social Work (LMSW or LCSW) Min Work Background: Case Management/Transition Managerial experience in an acute care hospital setting required.
