What You Will Do: University of Maryland Shore Regional Health is the principal provider of comprehensive health care services for more than 170,000 residents of Marylands Eastern Shore. UM Shore Regional Health operates 3 hospitals, a freestanding emergency center, a skilled nursing and rehabilitation center, outpatient care centers and home care services throughout the region. Be Part of Something Greater Responsibilities/Tasks Systematically and continually collects data regarding the health status of the client. The data is recorded, accessible, and communicated to appropriate members of the Multidisciplinary Healthcare Team utilizing system policies, procedures, and practice guidelines. Coordinates a comprehensive individualized Plan of Care for the client, their significant other, and healthcare provider to obtain expected outcomes. Implements the Plan of Care in accordance with established policies, procedures, and practice guidelines in a safe and appropriate manner.
