Consumer Directed Care (CDC) Skills Trainer - (Lynn, MA)

Duties: Provide program overview to interested consumers and their surrogates. Responsible for intake and orientation for new CDC referrals and determining eligibility; carry a caseload of consumers who are enrolled in the CDC service model. This position will also be responsible for assisting with marketing the CDC service model in and around the Greater Lynn area. This includes providing presentations to small groups of people, as well as individual presentations, and other activities specific to growing the model as needed. Requirements: Bachelor's degree. Prior background in Human Services, preferably with Elders. Outstanding written, verbal, interpersonal and technical skills necessary. Multi-lingual expertise preferred (Khmer or Spanish). Excellent time management and management skills are a must. Valid Massachusetts Driver's License required. Knowledge of Community Resources preferred. Hours: Part-time, twenty hours per week (Monday - Friday)
