The MDS Coordinator position is responsible for the day to day activities of the MDS Program. This position will report to the MDS Program Manager and Pharmacy Site Manager. This position is responsible for building and maintaining positive relationships with the Funding Source, Referral Source, Community Mental Health Staff, Community Partners and Pharmacy Staff.
