RN Case Manager - (Eugene, OR)

Umpqua Health is looking for a RN Case Manager to work remotely. This position provides nurse case management services to Umpqua Health Alliance (UHA) Oregon Health Plan (OHP) enrollees and Medicare clients. Nurse case management duties include coordinating with providers, pharmacies, durable medical equipment suppliers, care facilities, hospitals, home health care providers, mental health agencies, chemical dependency agencies and other community services to provide care and case management for members. Current Oregon RN license in good standing and 3-five years of clinical background required; previous background in case management preferred. Possible telecommute options.
