POSITION SUMMARY Responsible for reviewing the status of the hospice patient with the IDT prior to conducting the required face-to-face visit as designated by the Hospice Medical Director; conducts a thorough comprehensive assessment of the patient in order to support evidence of decline and a continued need for hospice if appropriate; completes appropriate documentation with 48 hours of visit; timely execution of e-scribe orders; provides education to clinical and other staff; mentors and serves as a clinical resource
