The Director of Rehab Case Management will supervise daily activities of the case management department as well as provide leadership with the interdisciplinary team to maintain and achieve desired independence outcomes. The director will be responsible for the coordination of care for all patients and promote effective use and monitoring of healthcare resources. The Director will also communicate and educate staff throughout the facility on the program’s performance, patient transition across the continuum, issue solving strategies, data support systems and clinical outcome data collection.
