Home Health Nurse Practitioner (NP) Flexible schedule! Farmington, ME 04938 There is a Home Health NP job in Farmington, ME 04938. The Home Health NP will conduct in home health assessments for a large Medicare advantage provider within Franklin county. While the average visit is 45-60 minutes, the nature visit can vary and could include performing an annual health assessment, a post discharge visit, or visiting more complex members more than once per year. The house call visits will be conducted on patients residing in Farmington, ME 04938 and/or the surrounding Somerset, Oxford, Androscoggin & Kennebec counties. Responsibilities: Travel to patient homes to conduct H&Ps, complete a medicare checklist, identify any gaps in care, follow up on post discharge visits etc. Work Hours: Flexible, create your own schedule Full-time or Part-time one year contract Preferred Background: one year of clinical background as a Nurse Practitioner (NP)
