Mid-Columbia Medical Center is seeking a RN Clinic Case Manager. This position is Full time and is eligible for a full benefits package. The RN Clinic Care Manager performs case management and disease management of specific chronic illnesses and complex care coordination with specialists, care givers, social services, insurance companies, discharge planners and others playing a part in health care delivery. The RN Clinic Care Manager also manages transitions of care and monitors ED utilization and follow up. The RN Clinic Care Managers may see scheduled patients for condition monitoring, case management, education assessment etc. Care Management is conducted via face to-face and telephone encounters. Performs RN responsibilities as defined by Standards and Scope of Practice for the Registered Nurse {(ORS. 678.150/851-###-#### through 851-###-####} Nurse Practice Act (2008)} and reports directly to the Clinic Nursing Operations Director.
