Clinical Manager, RN - Home Health - (Fall River, MA)

Summary: Under the direction of the Branch Director and within established agency policy and procedure, the Clinical Manager is responsible for the assessment, coordination, supervision and evaluation of all Clinical Department activities. Responsibilities: -Provides superior customer service to internal and external clients, customers and patients. -Accountable for the care and outcomes that his/her staff is providing. -Meets weekly with case management clinicians to review patient caseloads, assessing the quality of care, coordination of all disciplines involved and appropriate allocation of services. -Mentors new clinicians. -Performs joint visits with clinicians as a means of assessing clinician expertise to manage patient care needs, leading to excellent patient outcomes and education of clinician in accurate assessment and documentation of assessment utilizing OASIS tool. -Analyzes patient data by clinician, noting trends and working with clinician to optimize patient outcomes.
