Job details: POSITION SUMMARY:The Pharmacist delivers pharmaceutical care through the provision of patient-centered clinical service, medication details, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy. Principal Accountabilities Compounds and dispenses medications accurately for delivery to patient care areas. Documents intervention, outcomes and performance improvement. Manages medication therapy/protocols as a part of physician-pharmacist-nurse team. Provides functional supervision to supportive personnel. Provides medication details and education to medical staff and patients. Requirements: MIN JOB REQUIREMENTS: Education: Graduate from a college of Pharmacy accredited by the American Council on Pharmaceutical Education. PharmD and Post Graduate Residency (PGY-2 in Oncology) are desirable. Licensed as a Pharmacist (R.Ph) in the state of California is required. Board Certified in Oncology preferred
