Grant Community Connection Specialist - (Reading, PA)

Overview This position helps support the Accountable Health Community Grant. Responsibilities The Community Connection Specialist will be responsible for helping patients and their families to navigate and access community services. The Community Connection Specialist will screen patients for unmet social needs, deliver a customized list of community resources, conduct interviews and develop action plans to address identified needs, and conduct follow up call to patients and community service providers. This is an entry level position that will support the Community Connection Project and administrative staff.
