Your research skills and knowledge of team:based care will help to create a successful model throughout our clinics and ensure that patients receive the medical care they deserve. Responsibilities Under the guidance of the Manager of Medical Home Support and Medical Home Medical Director, identifies, defines and develops process flows and procedures in support of various functions within the clinic setting. Coordinates deployment of process flows, procedures, and best practices to all Legacy Clinics.
