Case Manager- HOME Program - (Kissimmee, FL)

Job Purpose: As a Case Manager at Aspire Health Partners, your job purpose is to provide overall assessment and linkage services for homeless families. These functions include but are not limited to providing screening, referral, emergency, crisis and case management services, documenting clinical records in Avatar and HMIS, working closely with the county and HSN while maintaining the integrity of the client, the understanding and expertise of the necessary technology to communicate effectively with staff and, on occasion, act as the authorized client transporter for necessary treatment and services. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures. Position Requirements: Expertise to receive and maintain a(n) * Level II Background clearance * Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire's insurance company.
