Social Work Care Coordinator The Social Work Care Coordinator is responsible under the supervision of the department management for providing patients and families with emotional support, advocacy as appropriate, financial info, discharge planning and social work services. This position also serves as a liaison between the medical staff and the patient/family and provides interventions designed to minimize patient stress and maximize coping skills to expedite recovery. Care Coordination is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options, services and alternative levels of care to meet an individual?s needs. By assuming a leadership role within the interdisciplinary team, the Social Work Care Coordinator promotes appropriate utilization, high-quality care and cost effective outcomes.
