Zero Pinellas Navigator - (Clearwater, FL)

Job overview Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning. The Zero Pinellas Navigator will act in a supportive role to medical providers and patients seeking treatment for Pre-Exposure Prophylaxis (Truvada as PrEP). The Zero Pinellas Navigator will provide linkage services to individuals for medical care and treatment for PrEP, including specialized HIV prevention education and sexual risk assessment. In collaboration with Zero Pinellas staff, the Navigator will provide linkage to resources according to individualized patient needs, identify and address barriers to care and treatment, and promote healthcare retention and positive health outcomes. This is a 3 year grant funded position.
