Supportive Housing Direct Care Staff II - (Taunton, MA)

Responsible for providing outreach/case management services to program clients and program service delivery including: Provide range of rehabilitation services; Provide range of support services; Provide range of housing development services; Ensure enactment of each clients Individual Service Plan (ISP) including completion of all required paperwork to verify homelessness; HMIS data input; Participate in daily organization meetings, ISP review meetings, and supervision, and training; Complete documentation in a timely fashion, including incident reporting; Other duties as defined by agency and/or program leadership Disclaimer: This position overview is only a summary of the typical functions of the job, not an exhaustive or comprehensive list of all possible job responsibilities, tasks, and duties.
