Apply Description Zepf Center is a local non-profit that provides behavioral health and vocational services to youth and adults with severe and persistent mental illness in Lucas & Wood Counties. Services include child & adolescent psychiatric, medical, substance abuse, residential, and therapy services, as well as career development. We are currently seeking a full time Adult Integrated Care Specialist (Case Manager). The purpose of this position is to provide a broad range of community support and coordination of services to people experiencing serious mental illness and to ensure their needs are addressed. The Integrated Care Specialist assists clients with the development of short and long term goals and acquisition of community resources. Services will be provided with respect and in accordance with client's rights.
