This and other social worker jobs brought to you by AlliedHealthJobCafe.com Overview Our Discharge Planner administers a coordinated program of medical and mental health discharge planning, in conjunction with the DOC's transitional planning process pursuant to 103 DOC 493 Reentry Policy, to ensure that each inmate who is in need of continued care at the time of release from custody is linked with appropriate community agencies that will provide such care. Requirements Education: Bachelor's degree in social work or closely related position from an accredited college or university.Background: Previous background in developing successful transition plans involving community resources for persons with mental illness. Background in social work case management in a mental health agency for mentally ill population.Licenses/Certifications: None required. Responsibilities Attends department release planning meetings and clinical case conferences.
