Care Coordinator - (Orlando, FL)

Job Purpose: As the Care Coordinator at Aspire Health Partners, your job purpose is to provide overall intensive case management. These functions include - but are not limited to - providing outreach services, life skills training, 24hr on-call emergency crisis intervention and constant support of the clients referred to the program and improve their chance of recidivism by maintaining a community based setting. In addition, the Care Coordinator must show competency in effectively engaging individuals undergoing the treatment process, expertise to construct a comprehensive treatment plan while maintaining documented assessments in compliance with DCF, CARF and Managed Care requirements and make themselves to on-call responsibility as necessary. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
