Patient Care Manager (Medicine/ Behavioral Health, RN) - (Washington, DC)
GENERAL SUMMARY Directs, supervises and provides holistic, culturally competent, quality and cost-effective evidence-based nursing care for patients/families. Applies the nursing process within the context of the organization's shared decision making frame work. Collaboratively manages the patient's transitions across the continuum of care, to assist patients or populations in achieving or maintaining an optimal level of health. Is recognized as a professional role model and clinical expert and promotes a professional environment that supports continuous learning. Delegates and supervises work appropriately. The Patient Care Manager supports, assists and reports to the Nurse Director. Can assume the role of Nurse Director in his/her absence. * EDUCATION: Bachelor's degree in Nursing (BSN) is required. Master's degree or Master's program enrolled preferred. * PRIOR BACKGROUND:Two Years of progressively more responsible job related skills.
