Home Health Scheduler - (Modesto, CA)

Job summary: The Scheduler position is the key position to coordinate staffing and provides timely and appropriate staffing for safe and effective care delivery. Optimally schedules PCC RNs, PCC PTs, Case Managers, Team RNs, Physical and other therapists, Home Health Aides, Medical Social Workers and Dieticians to accomplish patient visits. Maintains coordination of the Weekend ,Holiday Staff Schedule, the On Call RN schedule and the HHAs daily assignments. Ensures that all Starts of Care, Resumptions of Care and Eval packets are distributed to the Rehab teammates and to the Team Care Coordinators for nursing assignments. Ensures that communications from patients, physicians and caregivers are relayed in a timely manner. Demonstrates effective liaison activities and interdepartmental communication and cooperation to support essential agency activities. Must be able to deal with a challenging workplace atmosphere from time demands and conflicting priorities.
