Spiritual Care Coordinator- per diem - (West Branch, MI)

HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy. Responsible to provide spiritual support to clients, families, and the hospice staff via phone contact, home visits and referrals to the client/family, local clergy. Assists with memorial services and activiites relating to spiritual and religious issues of client, family and interdisciplinary team. Educational Qualifications Bachelors from an accredited institution of theology or equivalent, or Masters in Counseling or Masters in Social Work with two units of Clinical Pastoral Education (CPE) required; Masters in theological studies or equivalent and completion of two units of CPE preferred.
